Job summary

Commonwealth Care Alliance is seeking an Integrated Care Clinical Manager for Massachusetts. The RN will provide monitoring, follow-up, and clinical care management to dually eligible enrollees with complex needs, coordinating with care teams to improve outcomes.

This mobile position requires travel to member homes and community settings while advocating for benefits under Medicare and Medicaid.

Qualifications

• Associates of Science degree in nursing from an accredited program.
• Current MA RN license and eligibility to work as an RN.
• Experience in care coordination or dual-eligible populations preferred.

Responsibilities

• Engage enrollees in homes and community settings to establish care management relationships.
• Function as liaison between providers, resources, and enrollees for seamless care.
• Lead the interdisciplinary care team to create holistic care plans.
