Avance Care in North Carolina is seeking a Referral Coordinator to support providers by submitting referrals promptly and closing the care loop with consult notes after visits. Qualifications

• High School Diploma or associate's degree in healthcare administration or related field preferred.
• Knowledge of medical terminology and HIPAA compliance preferred.
• 1 year of related experience preferred.

Responsibilities

• Ensure referrals are submitted, accepted and followed up on in a timely manner
• Provide appropriate clinical information to outside entities, as needed
• Ensure complete and accurate registration for patients
• Assist patients with referral issues and advocate with carriers for timely approvals
• Maintain detailed referral documentation and EMR communication
• Establish relationships with providers and outside entities
• Review details and expectations about the referral with patients
• Ensure chart is up to date with consult information
