Job summary

Southeast Integrated Care is seeking a Referral Specialist to serve as system navigator and primary contact for clients, families, referral sources, payers, and providers. The role ensures access to services, coordinates scheduling, and communicates with all parties involved in intake and ongoing care.

The ideal candidate will have a relevant degree and MH/DD/SAS experience, with strong communication and organizational skills. This is a full-time position located in Pembroke, NC.

Qualifications

• Bachelor's degree in Human Services or a related field preferred.
• 2–4 years of MH/DD/SAS experience with population, depending on degree.
• Strong communication and leadership abilities.

Responsibilities

• Serve as system navigator and primary point of contact for clients, families, referral sources, payers, and providers.
• Capture all referrals in EMR and coordinate scheduling of services.
• Communicate care plans and outcomes with referral sources and care teams.
• Assist with problem solving for barriers to care and coordinate community resources.
• Attend community events to build partnerships and awareness.
