Job summary

Alpharus Health is seeking a Re-Entry Specialist (Hybrid) based in St. Louis, MO, with a focus on coordinating services for clients transitioning back into the community.

You will work across treatment teams and community partners to assess needs, arrange resources, and monitor progress in a dynamic correctional healthcare setting. The role requires a Bachelor's degree in a related field or equivalent experience, plus relevant counseling credentials and strong interpersonal skills to support

Qualifications

• Bachelor's degree in psychology, social work, mental health counseling, or substance abuse counseling; 100 hours of clinical training.
• Two years in substance abuse field with case management experience.
• Current certification as an alcohol, drug, or substance abuse counselor as required by state.
• Advanced interpersonal skills to support clients through recovery; interact with family and external agencies.
• Analytical abilities to assess needs, resources, progress and prepare reports.
• Typing ability and working knowledge of word processing software to complete forms and reports.
• Valid local driver's license and being at least 21 years old.

Responsibilities

• Conducts client social, financial, medical and environment status and needs assessment.
• Coordinates with treatment team and community agencies to obtain resources and services for recovery.
• Communicates with treatment team, supervisor, probation and parole officers, and agency representatives to plan and coordinate services.
• Performs case monitoring and completes case action forms and documentation.
• Serves as a resource to treatment staff in resolving client needs and reducing recidivism.
• Duties vary by Center assigned.
