Action Wellness in Philadelphia is seeking a Prison Linkage Program Case Manager to deliver comprehensive case management to individuals in or recently released from the Philadelphia County Prison system. This role focuses on reducing reoffending, improving health outcomes, and fostering long-term stability through coordinated services.

You will work across prisons, clinics, and community settings, handling 40–45 HIV-positive clients, conducting intake, updating care plans, and collaborating

Competenze

• Bachelor’s degree in Social Work or related field.
• 2–3 years of case management, social services, or healthcare experience.
• Experience with justice-involved individuals and HIV/AIDS care preferred.

Mansioni

• Conduct in-person needs assessments to identify client strengths and service needs.
• Manage referrals to connect incarcerated individuals with appropriate services.
• Maintain up-to-date medical and demographic information with informed consent.
• Develop and update service care plans at six-month intervals.
• Provide case management, advocacy, and care coordination for HIV-positive clients during incarceration or reentry.
• Coordinate hospital discharges and arrange home care services.
• Ensure timely intake and documentation of client interactions and referrals.
• Attend team meetings and supervision sessions as required.
