Groups Recover Together is seeking a Recovery Support Coordinator in Knoxville, TN. This role supports 160–180 members with health-related social needs, treatment engagement, and coordinated care across clinical and community settings.

Qualifications

• Requires background in recovery-oriented care and case management.
• Experience with care coordination and working with SUD/mental health challenges preferred.
• Must meet state-specific background screening and pre-employment requirements.

Responsibilities

• Collaborate with a multidisciplinary care team to ensure high-quality, patient-centered care.
• Build trusted relationships with members to support recovery goals.
• Assist members in identifying goals, coaching, and engagement in recovery activities.
• Support re-engagement of unengaged members and transitions to care.
• Provide in-person, virtual, and telephonic support based on needs.
• Maintain a shared caseload and coordinate service delivery.
