Job summary

Alpharus Health is a national nonprofit behavioral health provider offering substance use and mental health treatment. We are seeking a Re-Entry Specialist for a hybrid schedule in Charleston, MO, combining four remote days with one onsite day weekly at Northeast Correctional Center.

The role focuses on comprehensive case management, coordination with treatment teams and community partners, and ensuring continuity of care for clients transitioning back into the community.

Qualifications

• Advanced communication skills; demonstrated knowledge of public welfare and social services; minimum of Bachelor's degree or equivalent education with 100 hours of clinical training.
• Ability to assess client needs and provide intensive case management with minimal supervision; knowledge of community resources; two years in substance abuse field with case management.
• Current certification as an alcohol, drug, or substance abuse counselor as required by a relevant agency in the employment location.
• Advanced interpersonal skills to support clients through recovery; interact with families and outside agencies.
• Analytical abilities to assess needs, provide case management, and prepare progress evaluations.
• Typing ability and ability to use word processing software to complete required forms and reports.
• Valid local driver's license and age 21+.

Responsibilities

• Conducts client social, financial, medical and environment status and needs assessments.
• Collaborates with treatment team and community agencies to obtain resources and services for recovery and treatment.
• Communicates with treatment team, supervisor, probation and parole officers, and agency representatives to plan and coordinate services.
• Performs case monitoring and completes case action forms and documentation.
• Serves as a resource to treatment staff in resolving case problems and reducing recidivism.
• Duties vary by Center assigned.
