Job summary

Alpharus Health is seeking a Re-Entry Specialist to support clients at the Northeast Correctional Center in Bowling Green, MO. You will conduct social, financial, medical and environmental assessments and coordinate resources with the treatment team and community agencies.

The position offers a hybrid schedule—four remote days and one onsite day per week—fostering collaboration, outreach, and recovery planning.

Qualifications

• Bachelor's degree in psychology, social work, mental health counseling, or substance abuse counseling is required or equivalent experience.
• At least 2 years of experience in the substance abuse field with case management.
• Current certification as an alcohol, drug, or substance abuse counselor as required by state.

Responsibilities

• Conducts client social, financial, medical and environment status and needs assessment.
• Works with treatment team and community agencies to obtain resources and services.
• Communicates with treatment team, probation and parole officers, and agency representatives to coordinate services.
• Performs case monitoring and completes case action forms and documentation.
• Resolves case problems and reduces recidivism through resource linkage.
