Job summary

Community Interactions, Inc. in Wilmington, DE is seeking a Program Coordinator to partner with the Residential Director and Team Leaders to manage daily programmatic operations for Delaware residential homes.

The role coordinates person-centered services for individuals supported, works with DSPs, supports planning meetings, and ensures compliance with DDDS provider standards through PCPs and THERAP documentation.

Qualifications

• Bachelor’s degree and 3–5 years’ work experience with individuals with disabilities.
• Knowledge of division rules, regulations and policies for care, treatment, and rehabilitation in community settings.
• Strong communication skills, oral and written; ability to read, write, and present.

Responsibilities

• Maintain Outlook calendar for home QA visits and quarterly DE QA audits.
• Interface with DDDS state offices and job coaches to ensure needed services.
• Attend PCPs and team meetings to monitor and respond to planned services.
• Train and mentor DSPs on documentation and program outcomes in THERAP.
• Review spending records and complete quarterly PSR reviews for individuals.
