Job summary

The Columbus Organization, LLC is seeking a Housing Specialist to coordinate care and prevent homelessness in Delaware. Hybrid role with statewide travel to Kent and Sussex Counties.

Responsibilities include case management, records maintenance, and liaison with agencies to secure housing resources. Requirements include a high school diploma with human services experience or an associate degree in a related field, plus experience in reporting and regulatory compliance.

Qualifications

• High school diploma with 3 years of human services related experience.
• Associate’s degree or higher in behavioral, social sciences or related field OR 1 year in health or human services support.
• Six months’ experience in case management with planning, development, and evaluation.
• Six months’ experience in making recommendations as part of service assessment.
• Six months’ experience interpreting laws, rules, regulations, standards, policies & procedures.
• Six months’ experience in narrative report writing.

Responsibilities

• Act as liaison with DAs, state agencies and community partners to support individuals with disabilities and brain injuries.
• Maintain confidentiality and HIPAA compliance.
• Maintain electronic case records and required forms per state standards.
• Represent the program and agency professionally and foster community relationships.
• Coordinate housing appointments and assess apartment suitability for clients.
• Provide weekly housing status reports and assist with subsidy program applications.
