Mercy Clinic Primary Care – Chesterfield Valley is hiring a Primary Care Nurse to support triage calls and patient flow. The role works with Mercy Primary Care Providers to deliver patient-centered care and drive quality outcomes for an attributed panel.

The position starts on-site with a minimum three months of on-site training, then shifts to a 50/50 hybrid model. RN or LPN qualifications apply with 2–3 years of experience preferred.

Qualifications

• Diploma in Practical Nursing for LPN track.
• Active LPN license (or RN license for RN path).
• 2–3 years of experience preferred.

Responsibilities

• Manage triage calls/messages for the clinic.
• Collaborate with Primary Care Providers and the care team.
• Work on-site during training with a path to hybrid work.
