Mercy Clinic Primary Care – Chesterfield Valley is seeking a Primary Care Nurse to support providers through triage calls and messages, ensuring timely, safe, and patient-centered care. The role involves on-site training followed by a hybrid work arrangement and regular on-site clinic meetings.

The position requires either an LPN with a diploma and active license or RN with an ASN/BSN or equivalent experience and active RN license, with 2–3 years of experience preferred.

Qualifications

• Diploma in Practical Nursing (LPN) is required.
• Active LPN license and 2–3 years of experience preferred.
• RN qualifications include AS/BSN or 4 years of commensurate experience with active RN license and 2–3 years of experience preferred.

Responsibilities

• Provide triage calls/messages for the clinic.
• Collaborate with Providers and clinical care team to drive quality outcomes.
• Work with Mercy Primary Care Providers to deliver patient-centered care.
• After training, transition to a 50/50 hybrid work model with on-site five days per pay period.
