Mercy Clinic Primary Care in Chesterfield Valley is seeking a Primary Care Nurse to partner with Providers and the care team, triaging calls and messages for an attributed patient panel. After a minimum on-site training period, the role transitions to a 50/50 hybrid schedule with on-site meetings monthly.

The position supports a Monday–Friday 8:00–16:30 schedule with no nights, weekends, or holidays, and emphasizes patient-centered care and Mercy Service Standards.

Qualifications

• Diploma in Practical Nursing required.
• Active LPN license required.
• 2–3 years of experience preferred.

Responsibilities

• Manage clinic triage calls/messages.
• Collaborate with Providers and care team to drive quality outcomes.
• Support on-site operations and scheduling, transitioning to hybrid after training.
