The State of Vermont is seeking a Permanent Supportive Housing Case Manager for Waterbury, VT. You will coordinate care plans, assess eligibility, and assist with Medicaid access within a collaborative team.

The role requires a strong commitment to housing first principles and trauma-informed care, with field and office duties. Hybrid work up to two days remote, and three days in Waterbury.

Formación

• Master’s degree in education, human services, health, counseling or related field.
• Bachelor's degree in education, human services, health, counseling or related field and one (1) year of experience or training in housing, human services, mental health, substance use, public health, education, counseling or related fields.

Responsabilidades

• Eligibility and Assessment: Determine eligibility for program services; conduct whole-person assessments of need, collaborate with participants to co-create person-centered care plans, and conduct annual reviews to assess progress.
• Medicaid Support: Assist with accessing and renewals for Medicaid.
• Service Coordination: Coordinate services with local contracted providers, ensuring that all plans and services align with participants' needs and priorities.
• Team-Based Care Coordination: Refers to and collaborates with a range of providers including, but not limited to, Designated Agencies, Preferred Providers, Primary Care Physicians, and the VT Chronic Care Initiative, utilizing the Team-Based Care model.
• Quality Assurance: Engage in quality assurance and management activities, including training and technical assistance to providers, site visits, case reviews, and other monitoring activities to ensure program adherence.
