CWS Burlington is seeking a Case Manager to provide direct services and holistic support to clients facing complex challenges. You will conduct assessments, develop service plans, coordinate referrals, and advocate within community systems to ensure trauma-informed, culturally responsive care.

The role emphasizes collaboration with partners, documentation accuracy, and ongoing professional development in trauma-informed practices, with hybrid or in-office requirements and some travel as needed.

Competenze

• Bachelor's degree or equivalent experience in social work, human services, psychology, public health, or related field.
• Minimum 3 years of case management or direct service experience, preferably in nonprofit/humanitarian settings.
• Strong understanding of trauma-informed, strength-based, and culturally responsive care.
• Excellent communication, documentation, and interpersonal skills.
• Ability to manage diverse caseloads and work under pressure.
• Knowledge of local community resources and social service systems.
• Proficiency in Microsoft 365 and case management software.

• Conduct intake assessments to determine client needs, risks, and eligibility for services.
• Develop individualized service plans with measurable goals and timelines.
• Provide ongoing case management, monitoring progress, and adjusting plans as needed.
• Facilitate access to housing, healthcare, employment, education, legal support, and financial assistance.
• Offer crisis intervention and safety planning as appropriate.
• Advocate for clients with external agencies to secure resources.
• Maintain accurate, timely, confidential case files and track client outcomes.

• Contribute to program development, improvement, and evaluation.
