Job summary

The Columbus Organization in Vermont is seeking a Case Manager to coordinate the application, referral, support planning, and monitoring processes in alignment with applicable policies and regulations. This is a hybrid role with required travel throughout Windham County in Vermont.

You will engage in assessment, planning, and evaluation activities, and support the individual in ISA planning, ensuring participation and clear communication across the support team.

Qualifications

• Bachelor's degree in a human services related field or equivalent and 1 year of related experience.
• Associate degree in human services or related field with 2 years of related experience.
• High school diploma with 3 years of human services related experience.

Responsibilities

• Coordinate the application, referral, support planning and monitoring processes in compliance with policies and regulations.
• Engage in assessment and planning activities for individual services and documentation.
• Assist individuals to invite participants to ISA planning meetings and develop planning agendas and notes.
• Support the individual through the entire ISA planning process and ensure information is shared with the support team.
• Review and update ISA and related documents at least annually with the individual.
• Provide information on self-direction opportunities, complaints, grievances and appeals, and connect to third-party assistance when needed.
• Coordinate meetings with other providers and ensure consent to share experiences is obtained as requested.
• Communicate service authorization status to the planning team and make referrals as needed.
