Job summary

GSMG Rural Health Clinic in Hermiston, OR is seeking a Care Coordinator to manage patient care across the continuum, coordinating with providers, patients, families, and community resources. The role includes developing care plans, facilitating authorizations, documenting in the EHR, and supporting back-office duties during staff shortages.

The position requires strong clinical knowledge, communication skills, and HIPAA compliance; a high school diploma and BLS are required, with MA/LPN

Qualifications

• Clinical experience in a medical office or outpatient setting.
• Experience with patient care plans and authorizations.
• Familiarity with Epic or other EMR systems.
• BLS certification required.
• High school diploma required; MA or LPN preferred.

Responsibilities

• Coordinate and monitor individualized care plans with care teams.
• Advise patients and families; schedule referrals and tests.
• Obtain prior authorizations for meds and procedures.
• Document in the EHR and communicate with care team.
• Support back-office duties during staffing shortages.
• Participate in quality improvement and care coordination initiatives.
• May work evenings/weekends as needed.
