Good Shepherd Health Care is seeking a Care Coordinator to join our outpatient team in Hermiston, OR. The role partners with providers, patients, families, and community resources to assess needs, develop care plans, facilitate authorizations, and ensure timely follow-up.

Qualifications

• High school diploma or equivalent required.
• BLS certification required; MA or LPN preferred but not required.
• Clinical experience in outpatient care and care coordination helpful.

Responsibilities

• Coordinate and monitor individualized care plans with providers and families.
• Arrange referrals, tests, and follow-up appointments to ensure timely care.
• Document progress in the EHR and communicate with the care team.
• Assist with patient education, authorization processes, and resource navigation.
• Provide back-office support during staffing shortages and assist with patient care tasks as needed.
• Participate in team huddles and quality improvement initiatives.
