Mercy is seeking a Part-Time Nurse Navigator working in a Hybrid model to guide patients through their healthcare journey, coordinate care across inpatient and outpatient settings, and support discharge planning and post-acute services. The role emphasizes education, collaboration with interdisciplinary teams, and quality improvement initiatives.

The Nurse Navigator will use EPIC for documentation, participate in care coordination activities, and act as a resource and mentor to the care team

Qualificações

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• Minimum of 2-3 years of nursing experience.
• BLS certification.
• Preferred: BSN.
• Preferred: ACLS certification.
• Experience in care coordination, case management, utilization management, or patient navigation.

Responsabilidades

• Coordinate care for assigned inpatient and outpatient populations.
• Guide patients through their treatment journey, serving as a primary resource and advocate.
• Provide patient and family education, including pre-operative preparation and follow-up care.
• Collaborate with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered care.
• Support discharge planning and coordinate post-acute services.
• Monitor patient progress and help improve outcomes, patient satisfaction, and continuity of care.
• Assist with length of stay (LOS) management and utilization review activities.
• Participate in quality improvement initiatives and implementation of best practices.
• Serve as a clinical resource and mentor to members of the care team.
• Maintain accurate documentation within the electronic medical record (EPIC).
