Job summary

University of Illinois Chicago is seeking a Medical Social Consultant (Care Coordinator) in Chicago, IL. The role supports families through comprehensive assessments, person-centered care planning, and resource coordination in a hybrid work setting.

You will perform home visits, coordinate with MCO teams, and participate in multidisciplinary meetings, with a schedule (M-F, 8:00 AM - 4:30 PM) and a salary range of $52,000 to $63,000 annually.

Qualifications

• Master's degree in social/behavioral science or related health field.
• Current IL Licensure as LSW or LCSW.
• Three (3) years of progressively responsible social work experience in medical/social services.

Responsibilities

• Perform active care coordination by completing comprehensive health assessments and developing person-centered care plans.
• Monitor 30-day updates to the care plan and assess status changes.
• Conduct in-person visits at home or in settings like schools or hospitals.
• Document thoroughly within 48 hours to ensure compliance of case records.
• Engage in Medicaid rounds and multidisciplinary meetings as needed.
• Coordinate with MCO teams and support discharge planning.
