Qualifikationen

• Monitor 30-day updates and adjust plans based on changing needs.
• Conduct in-person visits at home, schools, or hospitals as needed.
• Document case records within 48 hours to ensure compliance.
• Engage with Medicaid managed care rounds and multidisciplinary meetings.
• Assist with discharge planning and resource coordination.
• Support families with insurance navigation, benefits, and DSCC applications.
