University of Illinois Chicago (UIC) seeks a Medical Social Consultant (Home Care Enrollment Specialist) in Chicago, IL. The role focuses on enrolling eligible children into DSCC Home Care Services and coordinating discharge planning with hospital and home care teams.

The Enrollment Specialist will assess eligibility, collect required data, and support families through the application process, while ensuring compliance with state and federal regulations. This is a hybrid, full-time position.

Qualifications

• Master's degree in a social or behavioral science field.
• Current IL LSW or LCSW licensure required.
• Three years or more of progressively responsible social work experience in medical/clinical or social services setting.

Responsibilities

• Assist families in applying for DSCC Home Care Services and explain programs and enrollment.
• Collect required medical and financial information to determine eligibility.
• Conduct home assessments to ensure safety and determine modifications needed.
• Coordinate available health care benefits and in-home nursing services.
• Assist families in interviewing and evaluating home care providers.
• Collaborate with hospital staff and DSCC teams to ensure timely discharge and service readiness.
