The position requires a Master's in social/behavioral sciences and IL licensure (LSW/LCSW) or comparable experience.

Qualifikationen

Master's degree in social or behavioral science or related health field.
Current IL LSW or LCSW licensure.
Three years of progressively responsible social work experience in a medical/clinical or social service setting.

Aufgaben

Introduce care coordination services and facilitate the in-home nursing application process.
Obtain documentation to determine eligibility for DSCC programs and services.
Assist families in collecting required medical and financial information for eligibility determination.
Conduct home assessments to ensure home safety and determine if modifications are needed.
Collaborate with hospital staff to ensure discharge readiness and services during transition.
