Job summary

Fletcher Rehabilitation and Healthcare Center is seeking a skilled MDS Hybrid Coordinator to oversee care plans and ensure residents receive high-quality nursing care. This full-time role blends onsite and remote work (3 days onsite, 2 days remote) and requires active nursing licensure and MDS process knowledge.

Key duties include coordinating care plans, auditing documentation, and supporting discharge processes while maintaining infection control and confidentiality of health information.

Qualifications

• Licensed registered nurse with current state license.
• CPR certification required.

Responsibilities

• Coordinate, manage and monitor the written plan of care for each Resident to identify needs and goals.
• Assist with discharge processes and coordinate nursing workflow to ensure timely care.
• Maintain infection control standards and safety precautions in daily care.
• Audit documentation for regulatory accuracy and completeness.
• Respond to hazardous conditions and supply issues promptly.
