Qualifications

• Registered Nurse (RN) license in good standing with the respective state board.
• Prefer 2+ years in post-acute, skilled nursing, or MDS-related roles.

Responsibilities

• Oversees and coordinates the MDS process in accordance with federal and state requirements.
• Leads the interdisciplinary plan of care team for resident assessments and care plans.
• Ensures accurate and timely completion of MDS and related documentation.
