Job summary

AltaMed Health Services Corp. is seeking a Licensed Vocational Nurse to develop and implement homecare services for program participants in California.

You will coordinate contracted home services, durable medical equipment, and nursing home care, and attend IDT and coordination meetings. Required: graduation from an accredited LVN program, active CA LVN license, at least 1 year of LVN practice, valid CA driver's license, and BLS certification.

Qualifications

• Graduation from an accredited LVN program and active California LVN license.
• At least 1 year of LVN practice in a healthcare setting.
• Experience with frail/elderly populations is highly desirable.

Responsibilities

• Develop and implement homecare services for participants.
• Coordinate contracted home services and durable medical equipment.
• Coordinate nursing home care and attend IDT/Coordination meetings.
