Job summary

Titanium Healthcare in California is hiring a Licensed Vocational Nurse (LVN) Care Coordinator to support the ECM program under CalAIM. You will work in a hybrid field-based role, spending most of your time delivering in-person care in the community and部分 remaining administrative tasks remotely.

Travel within the service area is required, with mileage reimbursement provided. You will collaborate with an interdisciplinary team to coordinate timely access to primary care and behavioral health

Qualifications

• LVN license is required and must be current.
• Minimum 2+ years of LVN experience in a healthcare setting.
• Valid driver’s license with clean driving record.
• Ability to travel and work in community settings.

Responsibilities

• Facilitate access to primary care and behavioral health services.
• Coordinate care with Titanium’s interdisciplinary ECM team.
• Perform in-person visits and documentation in EHR.
• Engage members, conduct motivational interviewing, and health education.
• Complete assessments per healthcare plan guidelines.
• Comply with policies and perform other duties as assigned.
