Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• RN licensure in Illinois or BS/MS in human services related field, or LPN with experience are required for IL Plan.
• Position is hybrid-remote with 50% travel to member homes.

Responsibilities

• Evaluate member needs and resources to plan best outcomes.
• Develop and monitor long-term care/service plans.
• Coordinate between member/family and care provider teams for access to services.
• Provide resource support for employment, housing, independent living, and other needs.
• Monitor plans, status and outcomes; adjust as needed.
• Educate members and families on procedures, services, and benefits.
• Suggest leadership improvements for cost-effective care.
