Job summary

Bluestone Physician Services in Minnesota is seeking a Care Coordinator to empower seniors to navigate their healthcare journey. You will manage DHS and CMS requirements for MSHO/MSC+ members, travel to member homes and facilities, and coordinate access to medical care, waiver services, and community resources.

The hybrid position offers 50–70% local visits and 30–50% remote work, with a schedule of 8:00 AM–5:00 PM Monday–Thursday and 8:00 AM–3:00 PM Friday.

Qualifications

• Current MN Licensed Social Worker (LSW, LMSW, LISW, or LICSW) – New grads encouraged to apply!
• Minnesota Registered Nurse license (RN) required or preferred depending on role.
• One or more years of experience with geriatric or disabled populations in case management/care coordination, Home Care, Group Homes, Nursing Homes, TCU or Assisted Living settings.

Responsibilities

• Coordinate face-to-face visits with members in their homes or facilities.
• Manage the Elderly Waiver program.
• Conduct annual PCA assessments and customized living tools for community members.
• Review health needs, set goals, and develop individualized care plans.
• Connect members with community and state resources and services.
• Support transitions of care and ensure safe discharge planning.
• Complete required documentation and collaborate with care teams.
