This full-time role combines on-site work in Wallingford with remote components and involves processing referrals, verifying insurance information, and initiating initial authorizations for Home Health and Hospice admissions.

The ideal candidate has a high school diploma, data-entry experience, and strong communication skills.

Qualifications

High school diploma or equivalent.
Experience with computer data entry, word processing, and reporting.
Valid driver's license and auto insurance.
Reliable transportation.
Experience with medical terminology or employment in a medical setting preferred.

Responsibilities

Process referrals and complete data entry of referrals.
Coordinate services for clients and families with staff and resources.
Assist with non-clinical tasks like answering phones, filing, ordering supplies.
Enter referral information into the health record.
Participate in staff orientation.
Collaborate with Intake Nurse on complex referrals.
Maintain intake department databases.
Work on special projects and communicate with referral sources.
Work weekends and holidays on rotating basis.
Perform other duties as required.
