Agate Housing and Services in Minneapolis is seeking a Case Manager II - Housing Stability to provide resident-centered case management for MPHA communities, building relationships and coordinating housing resources.

You will support eviction prevention, connect residents to income supports, healthcare, food, and services, and document outcomes while working with a team and respecting confidentiality.

Qualifications

• Bachelor's degree or minimum 2 years of related experience.
• Ability to build effective relationships with residents of diverse ages, backgrounds, and needs.
• Strong problem-solving, communication, advocacy, and conflict-resolution skills.
• Ability to manage multiple priorities, work independently, and collaborate with a team.
• Proficiency with Microsoft Office and ability to learn case management systems.

Responsabilités

• Build trusting, professional relationships with residents through outreach and check-ins.
• Develop, implement, and update individualized housing stability plans with residents.
• Provide eviction prevention and housing stabilization support and connect to resources.
• Assist residents with benefits and referrals for income supports, healthcare, food, and transportation.
• Coordinate with MPHA property management to address concerns while maintaining confidentiality.
• Provide crisis response, de-escalation, and referrals to services as needed.
• Educate residents about lease obligations and tenant rights.
• Collaborate with Agate colleagues and community providers to connect residents to services.
• Deliver services at assigned properties and through home visits as needed.
• Complete accurate case notes and meet reporting requirements.
• Participate in team meetings and cross-coverage across MPHA properties.
• Perform other related duties as assigned.
