Job summary

Agate Housing seeks a Case Manager II - Benefits & Eligibility to support MPHA residents in Minneapolis. You will provide resident-centered services, help with benefit applications and certifications, and connect clients to income supports and community resources.

Collaborative work with MPHA staff and partners is essential. You will maintain confidentiality, complete accurate case notes, and travel between properties to assist residents with barriers to housing stability and access to essential

Qualifications

• Bachelor's degree or 2+ years' experience in a related role.
• Experience providing person-centered services and navigating systems.
• Ability to manage several priorities with accurate documentation.
• Proficiency with Microsoft Office products.
• Excellent problem solving and conflict resolution skills.
• Valid MN driver's license and ability to pass background checks.

Responsibilities

• Provide resident-centered case management to MPHA residents, focusing on benefits access, income stability and housing stability.
• Meet with residents to identify needs, strengths, goals, and eligibility for public benefits and community programs.
• Assist residents with benefit applications, renewals, recertifications and income-change updates, including document gathering and deadlines.
• Help residents understand notices and connect to the appropriate agency, provider or advocacy support.
• Develop individualized service goals and follow-up plans with warm referrals and coordinated handoffs.
• Provide on-site hours, outreach, and individual meetings at MPHA properties, including in-home visits when appropriate.
• Support residents in addressing barriers affecting housing stability (income, food, transportation, utilities, health coverage).
• Educate on tenant responsibilities/rights and connect to financial stability and community services.
• Build relationships with benefit agencies, community organizations, and service providers to improve access.
• Travel between MPHA properties and perform other duties as assigned.
