Qualifikationen

• Bachelor's degree or 2+ years in case management, benefits navigation, or related field.
• Experience assisting individuals or families with public benefits and health coverage.
• Ability to explain complex health coverage and waiver information clearly.
• Proficiency with Microsoft Office products.
• Must have valid MN driver's license and pass background check.

Aufgaben

• Provide individualized case management and benefits navigation for MPHA residents.
• Assist with health coverage and benefit applications, renewals, and verifications.
• Coordinate MnCHOICES and waiver referrals and liaise with county staff and providers.
• Support residents in navigating waiver pathways and family-specific resources.
• Maintain thorough case notes and ensure confidentiality.
