Job summary

Agate Housing is seeking a Case Manager II - Housing Stability to provide housing-focused, resident-centered case management in MPHA communities in Minneapolis. You will build relationships, identify barriers, coordinate resources, and support residents to maintain housing and reach self-directed goals.

Assigned to MPHA AMPs, responsibilities include assessments, plan development, eviction prevention, benefit navigation, and close collaboration with property management and community providers.

Qualifications

• Bachelor's degree or 2+ years of experience in case management, housing, or human services.
• Ability to build relationships with diverse residents and service needs.
• Strong problem-solving, communication, advocacy, and conflict-resolution skills.
• Proven ability to manage multiple priorities and maintain documentation.
• Proficiency with Microsoft Office and data/case management systems.
• Willingness to travel to MPHA properties and meet driving requirements.

Responsibilities

• Build trusting, professional relationships with residents through outreach and check-ins.
• Assess needs, develop and update individualized housing stability plans.
• Provide eviction prevention and housing stabilization support and referrals.
• Assist residents with benefits, income supports, healthcare, and other services.
• Coordinate with property management to address concerns while maintaining boundaries.
• Provide crisis response, de-escalation, and referrals as needed.
• Educate residents about lease obligations and tenant rights.
• Collaborate with colleagues and community providers to connect residents to services.
• Perform home visits and community-based outreach as assigned.
• Complete case notes and documentation; meet reporting requirements.
• Participate in meetings and cross-coverage across MPHA properties.
• Perform other related duties as assigned.
