Qualifications

• Strong clinical foundation in nursing and medicine for hospice/palliative care.
• Experience coordinating interdisciplinary services and managing patient caseloads across care settings.
• Proficient in discharge planning and clear communication of ongoing needs and resources.
• Understanding of utilization management, regulatory compliance, and documentation standards.
• Active, unrestricted RN license in Oregon; hospice/palliative care experience preferred.
• Excellent communication, collaboration, and interpersonal skills for supporting patients and families.
• Ability to work independently in the field and manage time effectively.
• Valid driver's license and reliable transportation for community visits.

Responsibilities

• Manage a caseload of hospice patients with comprehensive nursing assessments.
• Develop and update individualized care plans and provide skilled nursing care in homes and facilities.
• Coordinate interdisciplinary services with physicians, social workers, chaplains, and other team members.
• Ensure timely documentation and participate in team meetings.
• Educate patients and caregivers on symptoms, medications, and care goals.
• Support end-of-life decision-making and advocate for comfort-focused care.
• Facilitate safe transitions in care settings and communicate changes in condition.
