We seek an Oregon-licensed RN with at least 2 years of home health experience, CPR certification, and a reliable vehicle. This full-time role offers growth, a stable schedule, and a supportive team environment.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license – Oregon.
• Minimum 2 years nursing experience (home health or public health preferred).
• Current CPR certification.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Perform comprehensive nursing assessments and develop individualized plans of care.
• Coordinate and manage home health services with physicians and care teams.
• Provide skilled nursing care, treatments, and patient education.
• Monitor patient progress and communicate changes to physicians.
• Implement preventive, rehabilitative, and restorative nursing interventions.
• Identify and coordinate discharge planning needs.
