Job summary

BeLoved Hospice is seeking a full-time RN Case Manager to join our team serving Salem, OR. You will perform patient visits, manage care plans, and coordinate with the interdisciplinary team to deliver compassionate, home-based hospice care.

The ideal candidate holds an Oregon RN license, is able to travel up to 75%, and has strong communication and organizational skills. A valid driver’s license and reliable vehicle, BLS certification, and ability to work with minimal supervision are preferred.

Qualifications

• Current RN license in Oregon.
• Prefer 2 years in public health, home care, or hospice.
• Strong communication and problem-solving skills.
• Ability to travel up to 75%.
• Valid driver’s license and insured vehicle.
• BLS certification.
• Ability to work with minimal supervision.

Responsibilities

• Initial and ongoing patient/home assessments to determine hospice needs.
• Develop and revise care plans with interdisciplinary team.
• Administer medications and treatments as prescribed.
• Coordinate care and communicate with physicians.
• Provide patient and family education and support.
• Maintain accurate clinical notes in the EHR.
• Ensure safety and quality of care.
