Role Description

The Hospice RN Case Manager is a full-time, hybrid role based in Albany, OR, with some work-from-home flexibility for documentation and care coordination. This role is responsible for managing a caseload of hospice patients, providing comprehensive nursing assessments, developing and updating individualized care plans, and delivering skilled nursing care in patient homes and facilities. The RN Case Manager coordinates interdisciplinary services, including physicians, social workers, chaplains, and other care team members, to ensure continuity and quality of care. Daily activities include symptom management, medication and treatment education for patients and caregivers, timely documentation, participation in team meetings, and proactive communication with families about care goals and changes in condition. The role also involves supporting patients and families through end-of-life decision-making, advocating for comfort-focused care, and facilitating safe transitions in care settings.

Qualifications

• Strong clinical foundation in Nursing and Medicine, with the ability to deliver skilled hospice and palliative care in home and facility settings.
• Experience in Case Management, including coordinating interdisciplinary services and overseeing a patient caseload across multiple care settings.
• Proficiency in Discharge Planning, ensuring safe transitions of care and clear communication of ongoing needs and resources.
• Understanding of Utilization Management principles, including appropriate use of services, regulatory compliance, and documentation standards.
• Active, unrestricted RN license in the state of Oregon; hospice or palliative care experience strongly preferred.
• Excellent communication, collaboration, and interpersonal skills, with the ability to support patients and families during end-of-life care.
• Ability to work independently in the field, manage time effectively, and use electronic health record systems for accurate, timely documentation.
• Valid driver's license and reliable transportation for community visits.
