UNC-Chapel Hill in North Carolina seeks a Medical Case Manager for the Infectious Diseases Clinic to assess patients, coordinate referrals, and determine eligibility for adults with HIV infection receiving outpatient care. You will link patients to medical and behavioral health services, transportation, housing, and food resources, working with a multidisciplinary team to improve health outcomes.

Bachelor’s degree in a Human Services field is required; health-care experience and EPIC proficiency

• Experience in health care settings.
• Accurate and timely documentation in electronic medical records (EPIC) or similar systems.
• Strong organizational, communication, and multitasking abilities.

Responsabilidades

• Provide medical case management, referral coordination, crisis intervention, and eligibility determinations for adults with HIV in outpatient Infectious Diseases Clinic.
• Coordinate access to medical, behavioral health, and community resources; assist with transportation, housing, and food security.
• Collaborate with licensed social workers, medical staff, nursing, benefits coordinators and community agencies to engage patients and improve health outcomes.
• Attend meetings and participate in team-based activities as part of an interdisciplinary group.
