Job summary

Titanium Healthcare is seeking a Lead Care Manager to oversee case management for ECM program members. This hybrid role involves field visits (70%) across South Bay and remote work (30%). The LCM will build care plans, coordinate with the care team, and advocate for patients to improve health outcomes.

Ideal candidates have 1+ year in healthcare or related fields, a valid driver’s license, BLS certification, and strong documentation skills. A supportive, patient-centered environment awaits.

Qualifications

• Minimum 1+ years of experience in healthcare, social services, or care coordination.

Responsibilities

• Manage an assigned caseload of adult and pediatric members.
• Conduct monthly in-person visits and assessments.
• Develop individualized care plans based on assessments.
• Coordinate communication among healthcare providers, social workers, and the care team.
• Monitor progress and update care plans per policy.
• Communicate changes to PCP and specialists as needed.
• Advocate for patients navigating the healthcare system.
• Provide education to members and families on health topics.
• Identify and connect members with community resources.
• Assist in discharge planning from hospitals or long-term care facilities.
• Participate in training new employees.
• Perform other duties as assigned.
