Heritage Health Network is seeking a Lead Care Manager to guide members through our Enhanced Care Management program in a hybrid role serving Orange County. You will coordinate care, develop personalized plans, and collaborate with providers and community resources to support member wellbeing.

In this role you will educate and advocate for members, identify gaps in care, and ensure timely documentation. This position emphasizes compassionate care and teamwork in a dynamic, community-focused

Qualifications

• Experience in care or case management (community-based preferred).
• Knowledge of Medi-Cal and ECM strongly preferred.
• Strong communication and interpersonal skills.
• Ability to work in a team-oriented environment.
• Willingness to travel within Orange County.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Serve as the primary point of contact for members and coordinate their care.
• Develop and manage personalized care plans with members and providers.
• Collaborate with healthcare teams and community resources.
• Support members through education, advocacy, and engagement.
• Identify and address gaps in care.
• Maintain accurate, timely documentation.
