Job summary

Little Masters Development Center in New York City is seeking an experienced Early Intervention Service Coordinator to support infants, toddlers, and families through the EI process. This hybrid role requires coordination of services, eligibility, and IFSP milestones while interfacing with families and providers.

Responsibilities include timely service delivery, resource navigation, and documentation, with a per-unit compensation model and a flexible schedule to support diverse family needs.

Qualifications

• Prior Service Coordination and/or Early Intervention experience is required.
• Knowledge of Early Intervention system including IFSPs, evaluations, eligibility, service planning, and transition procedures.
• Strong organizational, communication, documentation skills.
• Ability to manage multiple cases, deadlines, and documentation requirements.
• Bilingual Service Coordinators are strongly encouraged to apply.

Responsibilities

• Support infants, toddlers, and families through the Early Intervention process.
• Ensure EI services are provided in a timely and coordinated manner.
• Inform families of rights, procedural safeguards, and available services.
• Coordinate funding sources and help families access resources.
• Process referrals, schedule initial meetings, and conduct screenings.
• Coordinate, schedule, and attend evaluations and assessments.
• Coordinate and participate in IFSP meetings and reviews.
• Coordinate with families, providers, evaluators, and notify meetings.
• Maintain thorough documentation per state regulations and EI requirements.
• Assist eligible children and families in accessing EI services and supports.
• Provide ongoing service coordination and connect families with community resources.
• Help families navigate EI and advocate for appropriate services.
