Job summary

Thrive By 5 in New York City seeks a Service Coordinator for the Early Intervention Program. The role involves providing continuous, interactive services to support eligible children ages 0-3 and their families in home-based and/or community settings.

Responsibilities include assisting families to access EI and related services, coordinating IFSP outcomes with family priorities, and ensuring timely delivery of services across Manhattan, Queens, Brooklyn, the Bronx and Staten Island.

Qualifications

• Bachelor’s degree in health and/or human services field.
• Bilingual preferred.
• Ability to communicate with clients and families via phone and email.
• Previous experience preferred.
• Case management experience preferred.

Responsibilities

• Assist parents of eligible children in gaining access to Early Intervention and other services identified in the IFSP.
• Ensure the IFSP outcomes reflect the family’s priorities, concerns, and resources; adjust as needs change.
• Coordinate the provision of all services that the child needs and/or is receiving.
• Facilitate the timely delivery of services.
• Continuously seek appropriate services and situations that benefit the child’s development during eligibility.
• Perform related duties as required.
