The Commonwealth of Pennsylvania invites applications for a County Caseworker 1 in Bedford County MH/ID. The role focuses on monitoring services, linking clients to supports, documenting progress, and developing individualized plans to promote independence.

Hybrid work arrangement may apply; full-time schedule typically 8:00 AM to 4:00 PM, with potential for remote work after probation. Training occurs on posting and eligibility requires appropriate credentials.

Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college coursework which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are within two months of graduating with a qualifying degree/credits.

Responsabilités

• Service Monitoring: Track services and evaluate how well they meet individual needs
• Resource Linking: Connect individuals to helpful programs, supports, and referrals
• Case Documentation: Maintain accurate records in required computer systems and charts
• Plan Development: Organize and lead meetings to create individualized service plans
• Crisis Support: Provide intervention during urgent or stressful situations
• Community Coordination: Work with agency staff, providers, and resources to support individuals
