Job summary

Bedford-Somerset DBHS is seeking a County Caseworker 1 to connect individuals with essential services, monitor progress, and advocate for self-determination. You will build rapport with consumers and families, link to community resources, and document outcomes.

The role emphasizes crisis support, direct contact, plan development, and collaboration with agencies, with a hybrid work option after probation and a standard 8:00 AM–4:00 PM schedule.

Qualifications

• Two years of experience as a County Social Services Aide 3 and 12 college credits in sociology/social sciences or a related field.
• A bachelor's degree with at least 12 credits in sociology/social welfare/psychology or related disciplines.
• Equivalent mix of experience and training meets the educational requirements.

Responsibilities

• Monitor, link, and evaluate services and referrals for consumers.
• Provide direct face-to-face case management and maintain productivity standards.
• Schedule and facilitate meetings to develop an individualized service plan.
• Enter and manage information in state databases and charts per regulations.
• Offer crisis intervention and coordinate assistance as needed.
• Collaborate with agency staff, providers, and community resources.
• Link individuals to transportation services or arrange transportation to appointments.
