Qualifications

• High School diploma with 1 year of related work experience.
• Interest in working with nursing facility residents transitioning to the community.

Responsibilities

• Support the Community Transition Liaison as a core team member.
• Accompany the Liaison on nursing facility visits as needed.
• Assist with collecting documents for public benefits and housing applications.
• Ensure case documentation meets program standards and regulatory requirements.
• Perform clerical duties and data entry in the agency system.
