Qualifications
- High School diploma with 1 year of related work experience and/or interest in working with nursing facility residents including older adults or persons with disabilities who are transitioning from an institutional to a community setting.
Responsibilities
- Serve as onsite point of contact in nursing facilities for residents, families, health care proxies, and NF staff related to transitions to the community.
- Coordinate transitions from nursing facility to the community with care planning teams and community resources.
