Job summary

Lutheran Family Services is hiring a Community Support Specialist to provide rehabilitative and support services to adults with primary mental health needs. The role operates in a hybrid environment, covering office, field, and community settings in Fremont, Nebraska and surrounding areas.

The position emphasizes integrated treatment planning, client advocacy, crisis management, and coordination with community partners.

Qualifications

• Experience with case management and crisis management preferred.

Responsibilities

• Develop integrated treatment team plans with clients and update every 3 months.
• Facilitate goals related to community living, daily living skills, and health.
• Discuss discharge goals and develop discharge plans.
• Conduct required assessments.
• Arrange transportation for clients to appointments.
• Develop crisis response and safety plans for clients.
• Coordinate with other services and advocate for clients.
• Maintain caseload and meet billing requirements monthly.
• Attend meetings, trainings, and supervision sessions.
• Document services per guidelines and participate in PQI initiatives.
• Abide by HIPAA and ensure confidentiality of records.
