Job summary

Lutheran Family Services in Lincoln, Nebraska seeks a Community Support Specialist to provide and coordinate rehabilitative and support services for adults with mental health diagnoses, focusing on promoting independence in daily living and community participation.

Responsibilities include developing treatment plans with clients and care teams, coordinating services, arranging transportation, documenting progress, and ensuring safety and confidentiality in accordance with program guidelines.

Responsibilities

• Develop with clients integrated treatment plans and update every 3 months.
• Facilitate implementation of clients’ goals across community living, daily living, and health needs.
• Discuss discharge goals and develop discharge plans.
• Conduct required assessments and document all services.
• Arrange transportation for clients to appointments within the community.
• Develop crisis response and safety plans and assess safety as needed.
• Coordinate treatment with other providers and services.
• Advocate for clients and collaborate with service partners.
• Maintain caseload while meeting monthly billing requirements.
• Attend meetings, trainings, and supervision sessions.
