Job summary

Ruby's Place, doing business as Restorative Pathways, seeks a Clinical Case Manager in Tracy, CA to provide integrated case management and clinical support for individuals experiencing homelessness. The role combines psychosocial assessment, care planning, crisis intervention, and coordination of community resources to stabilize housing and well-being.

Qualifications

• Master’s degree (Case management, MA/MS in counseling) or field experience in clinical case management.
• Crisis intervention, case management experience with victims of trauma.
• Trauma-informed care training and crisis intervention experience.
• Experience in domestic violence, human trafficking, or homelessness settings.
• Valid CA driver’s license, proof of insurance, and approval to drive agency vehicles.
• Ability to pass background checks and fingerprinting as required.

Responsibilities

• Assess medical, behavioral health, and social needs of clients.
• Create care plans and coordinate transportation and appointments.
• Coordinate with medical providers and obtain signed releases.
• Provide behavioral health linkage and harm-reduction strategies.
• Deliver confidential, trauma-informed, client-centered services.
• Document outcomes and maintain client records.
• Complete trainings and timesheets; participate in supervision.
• Other duties as assigned.
